ADA request for accommodation process

Use this process to request a reasonable ADA accommodation or to address a facility access issue.

  1. Review the Reasonable Accommodation Guidelines.
  2. If you have a qualifying impairment/disability, first, speak with the principal or supervisor regarding your needs.
    Note: Worker's Compensation issues must be addressed with the Risk Management Department.
  3. If you and the supervisor agree on a reasonable accommodation, work with your supervisor to complete item #5 and send the completed forms to the EEO Office.
  4. If the supervisor is unable to provide a reasonable accommodation for you, work with your supervisor and complete/review items #5-9.
  5. ADA forms: Both forms must be completed and submitted to the EEO Office:
    1. ADA Request for Reasonable Accommodation Form
      This form will help your supervisor and/or the district's ADA Committee assess your specific disability or impairment and decide on a reason accommodation.
    2. ADA Health Care Provider Form (see last page of the ADA Request for Reasonable Accommodation Form)
      Medical documentation must be submitted from your health care provider and should clearly:
      1. Identify the qualifying medical condition;
      2. Specify the impact of the diagnosed medical condition to major life functions, including working and learning.
      3. Explain how the medical condition restricts or limits work performance or learning.
  6. Submit the completed forms to the EEO Office. Once your completed forms are received, you will be contacted by EEO Staff.
  7. Within 30 working days of receiving your ADA request form and supporting medical documentation, your request will be reviewed by the district's ADA Committee. You and your supervisor will be notified of the ADA Committee's determination. The district's ADA Committee meets monthly throughout the school year.
  8. Persons wishing to appeal an ADA Committee determination may do so by following the district's Citizen's Complaint Process. Citizen's Complaint Forms are available on the district's website.
  9. All ADA accommodation equipment and devices, including assistive technology & technological enhancements, are the property of the Anchorage School District and must be returned by the employee to the EEO Office upon leaving employment with the district.
